The people who have described either "current issues", or unexpected Death of their TV's have almost all listed 'tranny trouble' as the problem.
Here's my first hint: Even if it isn't shown in your maintenance schedule, I feel that
"There Ain't No Such Thing as A Lifetime Tranny Fluid." Replace every 50-70K miles. (50-60K for the first change, because the break-in period is tough on the fluid; 60-70K thereafter. Maybe even more often, if you tow in extremely hot weather.)
And here's my second hint, although it's a bit long to read and subtle to understand. It's with regard to automatic transmissions (and applies to all TVs and SUVs equiped with automatics, not just the Toyotas in the title):
